Health insurance doctor visit definition?

I’m looking at shape insurance plans and I see both a copay amount (25%) and an ‘personnel visit’ amount ($25). I’m trying to figure out where the line is drawn between routine personnel visits and ‘emergency’ or whatever you want to call it visits where I’d copay. Hopefully this is clear enough and somebody can give me a good resolution. Alѕο, any information on what shape insurance you have and the amounts of copay, personnel visit charges, deductibles, and premiums you pay would be greatly valued.
